FERPA Release form : Use this form to authorize or restrict the release of your educational records to others. Electronic Form.
Financial Aid Authorization: Authorizes federal financial aid to cover expenses such as books and uniforms.
Graduation application: Form to be filled out by all students planning to graduate.
2026-2027 Payment Plan Contract: All students who need to make payment arrangements should complete this contract with the business office prior to classes starting that semester.
Readmission Request:To be used by a student who is seeking readmissions to Salina Area Technical College.
Request for Accommodation and Academic Support: To be used for students who are requesting accommodation and academic support during their attendance at Salina Area Technical College.
TB Questionnaire: To be completed by ALL students before class attendance at Salina Area Technical College.
TB Additional Information Screening. To be completed by students who answered Yes to TB Questionnaire and needs to complete a TB Screening. This form can be taken to any health department or medical facility that will evaluate the student for tuberculosis.
